This form is only for clients who already have a prescription for a medication or food with our clinic. Please note that if the medication/food is close to running out, please call our clinic directly to expedite the request. If this is for a pet that our clinic hasn’t seen before or for a medication/food that your pet has not been prescribed by a veterinarian at our clinic before, then you can call our clinic to make an appointment or fill out our online appointment request form.
